To help us provide the most complete assessment, we recommend:

• The resident be present whenever possible. 
• A family member attend if practical, either in person or virtually. 
• Pets be secured if necessary. 
• Provide access to all areas you would like evaluated. 

If the resident has recently returned home from the hospital or rehabilitation, please let us know during booking.

 Your appointment typically includes:

• A conversation with the resident and family 
• A systematic walkthrough of the home 
• Observation of safety, accessibility, and property conditions 
• Photographic documentation, when appropriate 
• Time to answer your questions 

Most assessments take approximately 1-2 hours, depending on the package selected and the size of the home.

• Suggested next steps 
• Recommendations for additional professional evaluation, when appropriate
