A Cornerstead referral may be appropriate when:

• A discharge or care transition is approaching
• Home maintenance appears to have been deferred
• Adult children are coordinating care from another location
• The family is considering modifications but does not know where to begin
• Household conditions fall outside the referring professional’s scope

The family receives:

• A structured home assessment
• Photographic documentation
• Plain-language explanations
• Prioritized findings
• Identification of concerns requiring specialist evaluation
• A report that may be shared with authorized professionals

Cornerstead does not:

• Diagnose medical conditions
• Determine clinical functional capacity
• Provide occupational therapy
• Replace a comprehensive home inspection when one is required
• Perform repairs
• Sell recommended modifications
• Make placement or discharge decisions

Cornerstead complements the work of healthcare and aging-services professionals by providing structured information about the home environment and visible property conditions.

Cornerstead does not profit from the repairs or modifications identified in its reports.

Families remain free to select their own contractors, clinicians, and service providers.
